Druid Bear Form Quest
Level 10, received from any druid trainer. The alliance and horde versions are pretty much the same except you talk to different people at some parts.
Night elf version:
Step 1:
Quest: Heeding the Call
Objective: Speak to Mathrengyl Bearwalker in Cenarion Enclave of Darnassus. After completion you will receive a spell called ¡°Teleport: Moonglade¡±, which will teleport you to Moonglade.
Step 2:
Quest: Moonglade
Objective: Use the spell "Teleport: Moonglade" to travel to Moonglade. When you arrive, speak with Dendrite Starblaze in the village of Nighthaven.
Step 3:
Quest: Great Bear Spirit
Objective: Seek out the Great Bear Spirit in northwestern Moonglade and speak to him, then go back to Dendrite Starblaze.

Step 4:
Quest: Back to Darnassus
Objective: Return to Darnassus and speak with Mathrengyl Bearwalker in the Cenarion Enclave. And after that you will receive bear form.
Tauren version:
Step 1:
Quest: Heeding the Call
Objective: Speak with Turak Runetotem on the Elder Rise of Thunder Bluff. After completion you will receive a spell called ¡°Teleport: Moonglade¡±, which will teleport you to Moonglade.
Step 2:
Quest: Moonglade
Objective: Use the spell "Teleport: Moonglade" to travel to Moonglade. When you arrive, speak with Dendrite Starblaze in the village of Nighthaven.
Step 3:
Quest: Great Bear Spirit
Objective: Seek out the Great Bear Spirit in northwestern Moonglade and speak to him, then go back to Dendrite Starblaze. The bear spirit and his location is shown in above screenshots. He is at the same location regardless of whether you are horde or alliance.
Step 4:
Quest: Back to Thunder Bluff
Objective: Return to Thunder Bluff and speak with Turak Runetotem on the Elder Rise. And after that you will receive bear form.
Some people have asked me how to leave Moonglade after getting there. Both horde and alliance have flight paths there, which I've noted on this map. The blue square is the alliance flight path, and red square is the horde flight path. Make sure you do not go near the tunnel at the bottom of this map (where the road leads to), as it is guarded by lv 40-50 furbolg mobs.
